112 ACİL AMBULANS HİZMETLERİNDE ACİL OLMAYAN DURUMLARIN ve SUİSTİMALLERİN NEDEN OLDUĞU EKONOMİK KAYIPLARIN DEĞERLENDİRİLMESİ

Abstract
112 ambulans hizmetleri kapsamında tüm hizmetlerin ücretsiz olması, sistemi kötüye kullananlar için yaptırım bulunmaması ve sistemin kullanılmasındaki kolaylığın suistimali gibi nedenlerden dolayı sistem içerisinde oluşan boşluğun neden olduğu ekonomik ve zaman kaybının ele alınması ve araştırılması amaçlanmıştır. Araştırma ile Çanakkale 112 ambulans hizmetleri örnek olarak alınmış ve acil olmayan durumlar nedeniyle sistemin ne ölçüde meşgul edildiği ve bu meşguliyetin neden olduğu kayıpların değerlendirilmesi yapılmıştır. Çalışma 2016 yılında hizmet verilen 50.508 vakanın çalışma istatistikleri değerlendirilerek yapılmıştır. 112 ambulans hizmetleri merkez ve ilçeler olmak üzere 2016 yılında 23 istasyon ve bu istasyonlarda aktif olarak çalışan 27 ambulans ekibi ile 112 hizmeti vermiştir. 112 ambulansları ile 2016 yılında toplam vakaların 98'i asılsız ihbar, 661'i başka araçla nakil, 321'i diğer, 834'i diğer ulaşılan, 1047'i görev iptali gibi nedenler ile 2965 tanesine ambulanslar tarafından hiçbir şekilde müdahale yapılmamıştır. Bu rakam tüm vakalar içinde %5,87'lik bir değere denk gelmektedir. Toplam vaka sayısını ambulans sayısına oranladığımızda ambulans başına yıllık 1870 vaka düşmektedir. Bu sonuçlara göre toplam ambulansların %5,5'i yani 27 de 1.5 ambulans yukarıda belirtilen nedenlerle gereksiz yere meşgul edilmiştir. Ayrıca bu vakalar için harcanan sürelerde göz önüne alındığında 2016 yılı toplam vaka süresi 20.403 saat olarak gerçekleşmiş bu sürenin 556 saati bu vakalar için harcanmıştır. Verilerden anlaşılacağı üzere sunulan 112 acil ambulans hizmetlerinde gereksiz ve suistimale bağlı kullanım olduğu görülmektedir. Bu durumun önüne geçilmesi ve sistem içindeki boşlukların gerekli düzenlemeler ile kapatılması gerekmektedir. Katkı payı uygulaması, cezai yaptırımlar ve ücret tahsili gibi işlemler ile 112 acil sağlık hizmetlerinin amacı dışında kullanılması engellenmelidir.

Abstract
It is aimed to investigate and investigate the economic and time loss caused by the absence of all the services within the scope of 112 ambulance services, lack of sanction for system abusers and misuse of the system due to reasons such as abuse. With 112 ambulances, 2965 of the total cases were not intervened in any way by the ambulances, 98 of the cases being groundless, 661 being transported by another vehicle, 321 being the other, 834 being the other being reached, 1047 being canceled. This figure corresponds to a value of 5.87% in all cases. When we compare the total number of cases to the number of ambulances, 1870 cases per year per ambulance falls. According to these results, 5.5% of the total ambulances, ie, 1.5 ambulances, were occupied unnecessarily for the reasons mentioned above. In addition, considering the time spent for these cases, the total case duration of the year 2016 was 20,403 hours and 556 hours of this time was spent for these cases. It is seen that there are unnecessary and abusive usage in the 112 emergency ambulance services that are presented to be understood from the data. This situation has to be avoided and the gaps in the system have to be closed with the necessary regulations. The use of contributions, penal sanctions, wage-earning and other emergency medical services should be prevented.
