AVRUPA’DA VE TÜRKİYE’DE RUH SAĞLIĞI HİZMETLERİNİN KARŞILAŞTIRMALI OLARAK İNCELENMESİ

Abstract

Bireylerin, ruhsal sorunlarının üstesinden gelmelerine yardımcı olmak ve hastalık nedeniyle kaybettikleri toplumsal rollerini yeniden kazanmalarını sağlamak amacıyla yapılan çalışmaların hepsi, toplum ruh sağlığı hizmetleri içerisinde yer alır. Dünya genelinde bu hizmetler; hastane temelli model, toplum temelli model ve toplum&hastane dengeli model olarak üç farklı modelde sunulmaktadır. Türkiye’de ruh sağlığı hastalarının tedavi ve bakım hizmetlerinde hem hastane temelli modelden hem de toplum temelli modelden yararlanılsa da bu hizmetler ağırlıklı olarak hastane temelli modelde sunulmaktadır. Ülkemizdeki duruma kıyasla, Avrupa ülkelerinin birçoğu, hastane temelli modelden, bireylerin kendi yaşadığı çevrede ruh sağlığı hizmetinden faydalandığı, toplum temelli modele geçiş sürecini tamamlamıştır. Gelir seviyesi yüksek olan ülkeler ruh sağlığı hizmetlerini daha çok önemsemekte ve bu hizmetlere daha çok bütçe ayırmaktadır. Türkiye’de 11 adet ruh ve sinir hastalıkları hastanesi, toplamda 4231 yatak kapasitesi ile hizmet vermektedir. Bazı Avrupa ülkeleri ile karşılaştırıldığında, ruh sağlığı hastalarına uzun süreli bakım sağlayacak psikiyatri yataklarının, Türkiye’de daha az sayıda olduğu görülmektedir. Ülkemizde ruh sağlığı alanında hizmet veren toplam hemşire sayısı 1.677’dir. Ruh sağlığı alanında hizmet veren insan kaynağına bakıldığında ise, Dünya Sağlık Örgütü verilerine göre: Türkiye, Avrupa Bölgesi içerisinde nüfusuna oranla en az sayıda ruh sağlığı ve hastalıkları uzmanına sahip ülkedir. Avrupa Birliği’ne üye 28 ülkede, 100 bin kişiye ortalama 16,8 psikiyatri uzmanı düşerken aynı oran Türkiye’de 3,80 civarındadır. Buna ek olarak Avrupa Birliği’ne üye 28 ülkede 100 bin kişi başına 72,9 psikiyatri hasta yatağı düşerken aynı dönemde Türkiye’de bu oran 6,1 civarındadır. Bu sonuçlara göre, ülkemizde ruh sağlığı hastalarına yönelik genel hastanelerdeki yatak sayısının arttırılması bir gereklilik olarak değerlendirilmektedir. Ayrıca, bu hastalara uzun süreli hizmet sunacak, yataklı bakım evleri ve merkezlerinin kurulması, ruh sağlığı alanında çalışan sayısının da arttırılması gerekmektedir. Bu kapsamda çalışmanın amacı, genel olarak ruh sağlığı hastalarına yönelik tedavi ve bakım hizmetleri sunumu konusunu incelemek ve bununla ilgili farklı kaynaklardan (OECD Sağlık İstatistikleri, Sağlık Bakanlığı/Sağlık İstatistikleri Yıllığı, Dünya Sağlık Örgütü) elde edilen istatistiksel veriler ışığında Türkiye ve bazı AB ülkeleri (İtalya ve İngiltere) arasında karşılaştırmalar yaparak önerilerde bulunmaktır.

Abstract

All of the services provided to help individuals to overcome their mental disorders and to regain their social life lost due to their illnesses are in the context of community mental health services. These services are provided worldwide along with three different models named as hospital-based model, community-based model and community & hospital-balanced model. Although both of the hospital-based model and the community-based model are being used in the treatment and care services of the patients with mental disorders in Turkey, hospital-based model is being preferred mostly for treatment of patients. To compare with Turkey, most of the European countries have accomplished to change their hospital-based model to community-based model which provides individuals to have their mental health treatments in their own environment. Countries with higher income levels have higher budget expenses for health services and show more concern to mental health issues. In Turkey, 11 mental and nervous disorders hospitals serve with 4231 hospital bed capacity in all. Compared with some of the European countries, it is clear that there are fewer psychiatric beds to provide long-term care for patients with mental disorders. The total number of nurses serving in the field of mental health in our country are 1677. According to The World Health Organization’s data, Turkey have the least number of mental health and disease specialists for population-based need, compare within the European Region, considering the human resources serving in the field of mental health. European Union with 28 member states have an average of 16,8 psychiatry specialists per 100 thousand people, while Turkey has only around 3,80 specialist per same number of population. In addition, there are 72,9 psychiatric patient beds per 100 thousand people in 28 countries of the European Union but this ratio is around 6.1 in Turkey for the same time period. According to these results, to increase the number of patient beds for mental disorder patients in common hospitals in our country can be regarded as a necessity. Additionally, it is necessary to increase the number of mental health employees and the establishment of nursing homes and care centers to provide long-term care for these illnesses. In this context, the aim of this study is to examine the treatment and care services provided for mental health patients in general and make recommendations by comparing Turkey and some of the EU countries (Italy and England) by means of statistical data (OECD Health Statistics, The Ministry of Health/Annual Health Statatistics, World Health Organization) obtained from different sources.
