Hemşirelik Öğrencilerinin Klinik Stres Düzeylerinin Ve Etkileyen Faktörlerin Belirlenmesi

Abstract

Bu araştırma tanımlayıcı ve kesitsel tipte olup, birinci sınıf hemşirelik öğrencilerinin klinik uygulamanın ilk günü yaşadıkları stres düzeyini ve etkileyen faktörleri belirlenmek amacı ile gerçekleştirildi. Araştırmanın evrenini, 2013-2014 ve 2014-2015 eğitim-öğretim yılları arasında İstanbul ilinde, bir eğitim ve araştırma hastanesinde bulunan hemşirelik lisans programının Hemşirelik Esasları dersinin klinik uygulama eğitimi kapsamında farklı üniversitelerin 238 birinci sınıf hemşirelik öğrencileri oluşturdu. Örneklemini ise, araştırma kriterlerini karşılayan ve araştırmaya katılmayı kabul eden 225 lisans birinci sınıf öğrencisi oluşturdu. Veriler; Öğrenci Soru Formu ve Pagana Klinik Stres Anketi (KSA) aracılığı ile toplandı. Araştırmanın uygulanabilmesi için kurum izni ve etik kurul onayı alındı. Elde edilen veriler, bilgisayarda uygun istatistiksel yöntemler kullanılarak analiz edildi. Öğrencilerin %95.6’ının 17-21 yaş arasında, %86.2’inin kız olduğu ve %75.1’inin klinik uygulama için kendini hazır hissettiği belirlendi. Öğrencilerin KSA toplam puan ortalaması 28.25±9.72, KSA alt boyutları puan ortalamasının ise; Tehdit 6.69±4.25, Mücadele 14.54±5.69, Zarar 2.95±3.15 ve Yarar 4.06±2.16 olarak bulundu. Klinik uygulama için hazır olduğunu hissedenlerin Mücadele ve Yarar alt boyut puan ortalaması, hazır olduğunu hissetmeyenlerden istatistiksel olarak anlamlı düzeyde yüksek saptandı (p<0.05; p<0.01). Öğrencilerin nispeten düşük düzeyde klinik stres yaşadıkları belirlendi. Klinik uygulama süresince yaşamaktan korktuğu deneyimler arasında “hastaya zarar verme”nin ilk sırada yer aldığı görüldü. Bu sonuçlar doğrultusunda; klinik ortama benzer öğretim alanlarında öğrencilerin kendilerini daha hazır hissetmelerinin sağlanması, kliniğin genel yapısı (fiziksel yapısı, klinik ekibi, kurallar) konusunda öğrencilerin bilgilendirilmesi ve desteklenmesi önerilebilir.

Abstract

The research was a descriptive and cross-sectional type and it was aimed to determine experienced the stress level and the factors affecting the first-class nursing students on the first day of clinical practice.The population of the study consisted of 238 first-class nursing students of different universities in the hospital for the clinical skills training of the Fundamentals Nursing of course in a training and research hospital in Istanbul in the 2013-2014 and 2014-2015 academic years.The sample consisted of 225 first year undergraduate students who met the research criteria and accepted to participate in the study. Data were collected by Student Questionnaire and Pagana Clinical Stress Questionnaire (CSA).The permission of the institution and approval of the ethics committee were obtained for the application of the study. The data obtained were analyzed using the appropriate statistical methods on the computer. It was determined that 95.6% of the students were between 17-21 years old, 86.2% were girls and 75.1% felt ready for clinical practice. The mean total score of the students was 28.25±9.72, and the mean score of the sub-dimensions of CSA; the Threat was 6.69±4.25, the Struggle was 14.54±5.69, the Loss was 2.95±3.15 and the Benefit was 4.06 ±2.16. The mean score of Struggle and Benefit subscale of those who felt ready for clinical application was found to be statistically higher than those who did not feel ready (p<0.05; p<0.01). It was determined that the students experienced relatively low levels of clinical stress. Among the experiences he/she was afraid to live during clinical practice, it was seen that “harm the the patient” was first. In line with these results; it may be suggested to make students feel more ready in the teaching areas similar to the clinical environment, and to inform and support the students about the general structure of the clinic (physical structure, clinical team, rules).
